If there’s one group in New York that doesn’t get thanked enough, it’s social workers. You deal with families in crisis, seniors who depend on you, people with disabilities who trust you, and Medicaid members who need structure and guidance. Your caseload is heavy, your phone never stops, and the system doesn’t always make it easy.

One area where we can make your job easier: transportation. Specifically, making sure your clients actually get to their medical appointments.

The Transportation Gap in Social Work

Social workers know better than anyone that keeping a medical appointment isn’t always simple. Elderly clients may not drive. Disabled clients may need wheelchair-accessible transport. Low-income clients may not be able to afford a cab. Clients with chronic conditions — dialysis, physical therapy, chemotherapy — need transportation three or more times a week, 52 weeks a year.

When that transportation breaks down, appointments get missed, conditions worsen, hospitalizations increase — and the downstream effects create more work for you.

What Social Workers Should Know About NEMT

  • Covered by Medicaid managed care plans — Most NYC Medicaid plans include NEMT with no copay for eligible members.
  • Wide appointment coverage — Doctor visits, dialysis, therapy, specialist visits, mental health, lab work, and more.
  • Must be scheduled 24–48 hours in advance — Standing orders are available for recurring appointments.
  • Social workers can schedule on behalf of clients — You don’t have to leave it to the client if they’re unable to navigate the process.
  • Wheelchair-accessible vehicles are available — Specify the need when scheduling.
  • Standing orders eliminate weekly calls — For dialysis patients and others with fixed schedules, one setup call covers an extended period.

Practical Checklist for Setting Up NEMT for a Client

  1. Confirm the client’s Medicaid plan (virtually all NYC plans include NEMT)
  2. Get the transportation scheduling number from the plan’s member services
  3. Schedule at least 24–48 hours before the appointment
  4. For recurring trips, request a standing order
  5. Note vehicle needs: ambulatory, wheelchair van, or stretcher transport
  6. Confirm round-trip pickup and return times
